4 Prt, Carbonless, Snap off, crash number...

Anybody?
I have a side project, that needs the following.. I've poked around, google, etc. No real luck.
It seems Commercial/sheetfed printers are super easy.. forms printers - not so easy to find.
PM me if you're interested or can help...

Specs:
10,000
4 part NCR
8.5x11.625 - .625" snap-out/snap-off stub; 8.5x11 when torn off

1st - white coated back
2nd - white coated front & back
3rd - canary coated front & back
4th - white coated front

Form has marginal text/wording, ie: customer's invoice, contact copy, retain for 5 years, etc...

Crash numbering, starting with 48701

I work an inhouse (Blue Cross Blue SHield Massachusetts) but we do have a lot of forms done by this company.

If you're still looking, RR Donnelley has a lot of forms plants all over the world, they bought Moore who had bought Wallace... lots of forms and labels plants I hear. (I'm at a commercial sheetfed shop personally)

Please, keep them coming.
I'll at least quote with anyone, plus - it's nice to have a longer list of vendors for any and all side projects in the future. Most printers are hungry.. :twisted:
And depending on how hungry, it might feed me a little better!

Another vendor

I work for a small print shop in massachusetts, this job is to big for us, it wouldnt be economical for us to a job that size in house, so we use:

Its called Short Run Forms, but they can handle that no problem, they are based out of new york and there quoting and reordering system is automated and very easy to use, just thought id throw it out there.

We use "Newtown cpc" they're in Virginia. This would be right up their alley.
