We ran into this problem early on in our move to an all PDF workflow. Subsequently we have aske our customers to save their pdf files with an additional 1" surround when saving from their native file.

eg.
If your document size is 8.125 x 10.75 we ask that when saving the postscript that the output size (Paper Size) be 9.125 x 11.75, centered, with bleed and crop marks on. Make sure the crop marks are offset at least
.125" as the default on most programs leave them to close to the trim. This way not only do we get bleed the customer can also see right away if the bleed has been included. Works well for us.
